Hiring: Regional Care Coordinator Company: AncoraTN Location: Nashville, TN Job Posted Time: 2026-09-10 13:14:56 Employment Type: Full-time / Hybrid Target Skills & Keywords : Care Coordination About the job Required Skills: •Facilitate and strengthen collaboration among service providers, nonprofits, healthcare providers, and other stakeholders across the assigned Tennessee region(s) to create, centralize and formalize a more coordinated anti-human trafficking network of care. •Promote increased identification and referral of all human trafficking victims, with an emphasis on underserved populations such as those in rural areas, victims of labor trafficking, and male victims. •Work to expand and enhance the availability of comprehensive services including wrap-around, trauma-informed case management services in rural and underserved parts of the region, helping victims navigate the complex challenges of their human trafficking experiences through improved provider capacity and resource alignment. •Ensure that all human trafficking victims across each region can access seamless, comprehensive, and individualized service and support by identifying service gaps, fostering partnerships, and advocating for system-level improvements. •Build, formalize, and maintain relationships with regional partners to support victim-centered approaches and trauma-informed practices in service delivery. •Support regional and statewide efforts in training, resource sharing, and protocol development to improve overall response to human trafficking in coordination with the Tennessee Human Trafficking Task Force. •Track regional care coordination efforts and report on regional progress, service gaps, and outcomes related to collaboration, identification, and service availability as required by organizational and ECM funding guidelines. •Participate in statewide or multi-regional meetings to align local efforts with broader Tennessee anti-human trafficking initiatives. Qualifications: •Initial and subsequent random drug screening is required as per agency policy. •A background check is required. •Must be willing and able to embrace and exemplify our core values. •Full-time, 8am-4pm (willing to work some evening and weekend hours, as needed). •This position is part of an on-call rotation for after-hours and weekend emergencies. •Travel throughout the Middle Tennessee region is required.
