Hiring: Population Health Tech (Lafayette) Company: Louisiana Blue Location: Lafayette, LA Job Posted Time: 2026-09-11 10:40:38 Employment Type: Hybrid Target Skills & Keywords : EMR, Electronic Health Records, Microsoft Office About the job Required Skills: •How You Contribute to the Company’s Mission In This Role •Support Population Health visits by gathering patient information, completing screenings, documenting in the EMR, and assisting providers in home, clinic, and telehealth settings. •Conduct patient outreach, including scheduling visits and follow-up calls to support patient engagement and improve health outcomes. •Perform clinical tasks such as obtaining vital signs and supporting screenings (e.g., IRIS, PVD, A1C, urine microalbumin) within scope of practice. •Coordinate care by scheduling preventive screenings and connecting patients with internal care management and support services. •Assist with operational workflows, including preparing provider materials, running reports, maintaining records, and updating system data. •Support Population Health initiatives, health fairs, and program activities to drive quality outcomes and patient compliance. Qualifications: •High school diploma or equivalent required •Valid Louisiana driver’s license and proof of automobile insurance •Strong communication skills, including the ability to interact effectively with patients and care teams •Proficiency in Electronic Medical Records (EMR) and Microsoft Office Suite •Demonstrated capacity to work independently, manage time effectively, and adapt in a fast-paced environment •Strong organizational skills with attention to detail •Certified or Registered Medical Assistant (CMA/RMA) credential •Operational familiarity with telehealth platforms and hybrid care delivery models •Demonstrated ability to multitask and prioritize in a dynamic environment •Perform other job-related duties as assigned, within your scope of responsibilities.
