Hiring: Peds LPN Home Care Nights - Baton Rouge, Company: UnitedHealth Group Location: Baton Rouge, LA Job Posted Time: 2026-09-11 11:08:14 Employment Type: Full-time Target Skills & Keywords : Care Coordination About the job Experience: •1+ years of experience working as an LPN Required Skills: •Provides high quality clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team, following all applicable infection control procedures •Documents patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record utilizing the agency's electronic visit verification program (as applicable) •Actively participates in the agency's care coordination process including timely reporting patients' needs and changes in condition, attending patient case conference, communicating effectively to the patient, family, physician, other clinicians, agency management staff, and case coordinator, community case managers (as applicable) •You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Qualifications: •Current and unrestricted Licensed Practical Nurse licensure in state of practice •Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation •Demonstrated capacity to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client •Current LPN Licensure in LA without restrictions •Demonstrated capacity to work flexible hours as required to meet identified client needs •Demonstrated capacity to manage multiple tasks simultaneously •Good communication, writing, and organizational skills •UnitedHealth Group is a drug - free workplace.
