Qualifications: •Active Nurse Practitioner or Physician Assistant license in Arizona or Florida. •Demonstrated capacity to meet clinician credentialing and background check requirements. •Valid driver's license and reliable transportation. •Demonstrated capacity to travel to patients' homes. •Strong clinical assessment, communication, and critical-thinking skills. •Comfortable working independently while collaborating with a multidisciplinary team. •Comfortable providing care both in person and through telehealth. •Passion for relationship-centered and value-based care for patients with chronic illnesses.
