Preferred associate degree in healthcare, business or similar with 1 year experience Required Skills: •Stationed daily at primary account to maximize market engagement, and referral review efforts. •Review and update CRM Dashboard for accurate reporting via Forecast and other communication platforms. •Respond to all incoming referrals on Careport and/or other referral platforms timely, daily, and throughout the day. •Provide definitive answers for all incoming referrals within an hour of receiving and/or by end of day, if appropriate. •Communicate/Collaborate with Business Development and Operations leadership to expedite any delays, barriers, and solutions in real time to expedite, definitive answers. •Partner cross-functionally with AD Concierge on daily updates regarding all pending referrals for Forecast. •Partner cross-functionally with Transitional Advisors on communication with incoming referral, and admissions on CarePort, and other referral platforms. •Timely PCC data input including patient demographics, resident representative, POA, and Guardian information.
