Hiring: Health-Related Social Needs Network Manager Company: North Sound Accountable Community of Health (ACH) Location: Bellingham, WA Job Posted Time: 2026-09-10 10:55:00 Employment Type: Contract / Remote Target Skills & Keywords : Care Coordination, Change Management, HIPAA, Medicaid, Medicare, Microsoft Excel, Microsoft Office, Microsoft PowerPoint, Project Management, Zoom About the job Required Skills: •Inform Strategy: Bring external-network intelligence and partnership insight into shared Hub planning, advising on long-term strategy while the Community HUB Manager owns operational metrics, staffing, and budget execution. •Align Referral Pathways: Design external referral pathways with HRSN network partners and align them with the Hub’s internal workflows (owned by the Community HUB Manager) to ensure seamless, closed-loop navigation. •Equip Staff on Network Resources: Provide subject-matter content on HRSN partners, resources, and referral pathways to support frontline staff training and social-screening protocols; the Community HUB Manager owns curriculum delivery and maintenance. •Support Complex Cases: Contribute network and resource expertise to interdisciplinary case reviews led by the Community HUB Manager for individuals with overlapping medical and social needs. •Cultivate and Steward HRSN Networks: Serve as the primary architect of the Hub’s external network, building formal partnerships and referral relationships with health systems, managed care organizations (MCOs), Area Agencies on Aging (AAAs), regional carceral facilities and reentry Targeted Case Managers (rTCMs), and other HRSN supports. •Participate in Statewide and Cross-ACH Efforts: Develop and maintain partnerships with other regional Hubs through CACH and other cross-ACH activities to expand collaboration, share learnings and practices, and align statewide care coordination efforts. •Contract and Onboard Providers: Negotiate service expectations, operational agreements, and workflows with local housing, food, and transportation providers. •Assess Regional Capacity: Conduct ongoing regional assessments to evaluate community partner capacity and identify service gaps. Qualifications: •Five years of experience or achievements that encompasses oversight of programs related to health and health improvement; facilitation of and planning with diverse stakeholders in complex environments; incubation of new coalitions or alliances; and management of competing interests to reach shared goals. •Years of experience with North Sound ACH will be given preference when considering experience of candidates; and Degree from an accredited college or university in a subject area related to health, education, project management, or another field related to the position may be considered in lieu of years of required experience. •Proven success navigating systems for Medicaid, Medicare, or historically marginalized populations, with comprehensive knowledge of regional social safety nets and public benefits. •Technical literacy with social referral platforms and their integration with client management systems. •Strong negotiation, communication, and relationship-building skills for managing external partnerships across executive and peer structures. •A valid driver’s license and proof of insurance are required, along with reliable transportation to attend meetings and events. •Final candidates are subject to a background check. •Commitment to work to advance equity, racial and social justice •Lead by example, maintain professionalism, and leverage communication strategies to promote effective organization change and culture development. •Work effectively and respectfully with staff, stakeholders, and community.
