Hiring: Field Based Community Health Worker Company: Optum Location: Montgomery, AL Job Posted Time: 2026-09-12 11:15:21 Employment Type: Full-time Target Skills & Keywords : Medicaid, Medicare, Microsoft Excel About the job Experience: •Knowledge of Medicaid and/or Medicare population •Knowledge of culture and values of community •Operational familiarity with the resources available in the community Required Skills: •Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, level of care •Utilize both company and community-based resources to establish a safe and effective case management plan for members •Locate and engage members in the community •Partner cross-functionally with patient, family, and healthcare providers •Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services •Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members •Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the healthcare team •Document all member interactions and referrals.
