Hiring: Field Based Community Health Worker Company: Optum Location: Montgomery, AL Job Posted Time: 2026-09-12 11:15:21 Employment Type: Full-time Target Skills & Keywords : Medicaid, Medicare, Microsoft Excel About the job Experience: •Knowledge of Medicaid and/or Medicare population •Knowledge of culture and values of community •Operational familiarity with the resources available in the community Required Skills: •Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, level of care •Utilize both company and community-based resources to establish a safe and effective case management plan for members •Locate and engage members in the community •Partner cross-functionally with patient, family, and healthcare providers •Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services •Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members •Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the healthcare team •Document all member interactions and referrals. Qualifications: •High School Diploma/GED •2+ years of field-based experience •2+ years of patient interaction in the healthcare field. •1+ years of experience with knowledge of the resources available, culture, and values in the community •Must reside within commutable distance to Montgomery and surrounding communities in Alabama •Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI) •Intermediate computer proficiency including MS Word, Excel and Outlook •Demonstrated capacity to travel locally approximately 75% of the time with reliable transportation, that will enable you to travel to client and/or patient sites within a designated area, with a current and non-restricted state of Alabama Driver's License and State-required insurance •Knowledge and/or experience with behavioral health or substance use disorders Compensation: •Flexible work environment (work from home / hybrid options) •Competitive benefits and rewards package •Paid Time Off which you start to accrue with your first pay period plus 8 Paid Holidays Interested candidates, please apply directly through the job posting on company's career page or try via AI auto apply on this platform.
