Hiring: Family Specialist Company: Postgraduate Center for Mental Health Location: Bronx, NY Job Posted Time: 2026-09-03 11:13:38 About the job Experience: •At least one of experience working in human services with individuals with serious mental illness and experience in work with families. •Must be outcome oriented in a fast paced environment with deadlines. Required Skills: •Conducts daily visits in the community, and subsequent progress notes and other required documentation in accordance with agency and OMH standards. •Completes Wellness Plans, Comprehensive assessments and other documentation as required. •Completes family needs assessments by identifying history, current relationships and supports, and immediate needs and recommend treatment objectives for the consumer’s service. •Participates in daily morning meeting, and informs ACT team in daily meetings and weekly clinical meetings around family issues and incorporate findings into service plans •Involves family and significant others in the consumer’s treatment with the ACT team and provide psycho-education and relapse prevention based on motivational interviewing and cognitive behavioral strategies during visits. •Act as advocate and liaison for consumers in relationships with family, support systems, landlords and other community supports. •Provide 24-hour crisis intervention on-call services on rotating basis. •Performs other duties, consistent with the goals and objectives of the program, as directed by Team Leader. Qualifications: •Master’s Degree required - LMHC, LMSW, LCSW, or equivalent in licensure in NYS.
