Performs psychosocial assessments including collections of historical data, mental status assessments, and suicide risk assessments. •Consults and collaborates with medical center staff to coordinate interdepartmental patient services. Develops comprehensive and realistic treatment plans based on patient's needs. •Provides appropriate short term follow-up to ensure linkage and stabilization. Provides advocacy and support to patients to secure treatment and services. •Engages families, treatment providers, and other parties active in the well-being of the patient as appropriate whenever possible. Provides technical support, assistance, and education to patients, families, and other patient service providers as needed. •Assumes the duties of charge person as required. •Assumes the duties of mobile outreach on-call as required. •Possesses thorough knowledge of all regulatory requirements for confidentiality. •Demonstrates strict adherence to providing patient confidentiality as evidenced by documentation and behavior according to department policy.
