Develops comprehensive and realistic treatment plans based on patient's needs. •Provides short term follow-up to ensure linkage and stabilization. Provides advocacy and support to patients to secure treatment and services. •Engages families, treatment providers, and other parties active in the well-being of the patient whenever possible.
