Narcan distribution, value-based initiatives, and attend trainings, meetings, community events and cross trainings as beneficial. •Participate as a member of multidisciplinary teams in efforts to support increased patient engagement and adherence to appropriate and person-centered treatment. •Convey information inter/intra agency; communicate professionally with team regarding changes and conflicts. •Develop relationships with community stakeholders to enhance overall awareness, adopt harm reduction strategies, reduce stigma, facilitate efficient client care to support client community and employment integration. •In collaboration with the multidisciplinary team, use standardized assessment tools, engagement process, person centered planning and harm reduction methods to offer patients vocational rehabilitation services and document all efforts in the clinical record. •Provide rehabilitation support and recovery-oriented services tailored to individuals (adults, children and youth) coping with substance use, psychiatric or emotional challenges and/or co-occurring mental health and substance use. •Facilitate skill development and therapeutic interventions that promote self-management and reduce risk of recurrence of mental health or substance use episodes. •Help clients improve daily living skills essential for successful functioning in home, school, work, and community settings Qualifications: •To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
