Required Skills: •Under the direction of a licensed social worker or registered nurse, will assist care coordinators with accessing appropriate resources, coordinating referrals as necessary, providing community and educational support, and serving as a community liaison through telephonic outreach or in-person visits in homes, provider offices, or public agencies. •Create connections between vulnerable populations and healthcare providers •Help patients navigate healthcare and social service systems •Reduce social isolation among patients •Determine eligibility and enroll individuals in health insurance plans •Provide culturally appropriate health education on topics related to chronic disease prevention, physical activity, and nutrition at the direction of the registered nurse •Advocate for underserved individuals or communities to receive services and resources to address social determinants of health Qualifications: •To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
