Hiring: Bilingual Health Navigator - PrEP Company: Legacy Community Health Location: Houston, TX Job Posted Time: 2026-09-16 22:26:51 Employment Type: Remote Target Skills & Keywords : EHR, Electronic Health Records, Microsoft Office About the job Experience: •3 - 5 years of relevant experience •5 years of relevant experience •1 year of relevant experience preferred Required Skills: •Conduct virtual outreach and engagement activities to identify, educate, and recruit individuals who may benefit from PrEP and other HIV prevention services •Provide remote patient navigation services to facilitate linkage to PrEP care, including scheduling appointments and coordinating with clinical teams •Deliver individualized health education on PrEP, nPEP, HIV prevention, medication adherence, and safer sex practices using evidence-based approaches •Support patient retention in PrEP care through ongoing follow-up, adherence support, and re-engagement strategies •Utilize motivational interviewing and goal-setting strategies to help patients overcome barriers and maintain engagement •Provide referrals and linkages to supportive services, including primary care, behavioral health, and community resources •Partner cross-functionally with interdisciplinary care teams to coordinate care and ensure a seamless patient experience •Document all patient interactions in the electronic health record (EHR) in a timely and accurate manner Qualifications: •High School Diploma or equivalent •Bachelor's Degree in a related field with a minimum of 1 year of relevant experience preferred •Demonstrated actions and activities with respect to HIV care and prevention •Provides examples of performing education to clients of diverse cultures and backgrounds in one-on-one and/or group settings •Previous experience with Electronic Health Records Systems (EHRS) •Demonstrated capacity to demonstrate core competencies of HIV and HIV Prevention within 3 months of hire •Excellent communication skills and customer service skills •Adept in decision making responsibilities •Working knowledge and understanding of statistics and graphical representation principles •Strong computer skills (MS Office) Compensation: •Flexible work environment (work from home / hybrid options) •Paid Time Off & Paid Company Holidays Interested candidates, please apply directly through the job posting on company's career page or try via AI auto apply on this platform.
