Hiring: Bilingual Field Community Health Worker - Las Vegas, NV Company: UnitedHealthcare Location: Las Vegas, NV Job Posted Time: 2026-09-10 10:46:16 Employment Type: Full-time Target Skills & Keywords : Medicaid, Medicare About the job Experience: •5+ years of case management experience •2+ years of experience working within the Community Health setting in a field-based healthcare role •2+ years of experience or knowledge of the resources available, culture, and values in the community Required Skills: •Assess, plan, and implement care strategies that are individualized by patients and directed toward the most appropriate, lease restrictive level of care •Create a positive experience and relationship with the members •Utilize both company and community-based resources to establish a safe and effective case management plan for members •Partner cross-functionally with patients, family, and healthcare providers •Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services •Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members •Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the healthcare team •Document all member assessments, care plan and referrals provided Qualifications: •High School Diploma/GED OR 5+ years of case management experience •Intermediate level of computer proficiency including Microsoft Outlook, Teams, and the ability to use multiple web applications •Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI) •Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service •Demonstrated capacity to travel locally up to 50% of the time •Must reside within Las Vegas, Nevada or surrounding areas •Valid Nevada driver's license and current automobile insurance with access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area •Community Health Worker (CHW) Accreditation •Solves routine problems on own. - Works with supervisor to solve more complex problems Compensation: •Flexible work environment (work from home / hybrid options) •Competitive benefits and rewards package Interested candidates, please apply directly through the job posting on company's career page or try via AI auto apply on this platform.
