Holds staff accountable for meeting program goals and delivering effective protective services while ensuring interventions align with the least restrictive environment appropriate to each client. •Coordinates communication and collaboration with community partners, clients, families, and internal staff to support program operations, strengthen service delivery, and maintain effective working relationships across disciplines. •Assesses the vulnerable adult's ability to secure long-term services to include but not limited to: redirecting funds, securing a guardian or conservator, filling out appropriate documents for emergency services and connecting to probate or family court. •Coordinates and serves as the liaison with the State Office of the Adult Advocacy Division and ensures that reports and paperwork are completed within time frames established by APS policy including but not limited to Critical Incident Reports, Medicaid Reports and Emergency Funds paperwork. •Assesses and supports clients’ access to long-term services, including facilitating financial resources, guardianship or conservatorship, emergency services, and coordination with probate or family court as appropriate. •Serves as liaison with the State Office and Adult Advocacy Division. Ensures required reports, referrals, and documentation are accurate, complete, and submitted within established timeframes, including Critical Incident Reports, Medicaid documentation, ACP, Mental Health, OIDD, and Emergency Funds. Qualifications: •A Bachelor's Degree in Social Work, Human Services, Public Administration or a related field and three (3) years experience in Human Services or Social Services Programs. •A Master's Degree or higher in Social Work, Human Services, Public Administration or a related field and one (1) year of experience in Human Services or Social Services Programs. •This application for employment with the SC Department of Social Services must be completed in its entirety. •A resume may be included; but shall not be substituted in lieu of the completion of this application in part or in whole.
